The Case Management Coordinator - Social Work is responsible for coordinating patient care services and resources to assure optimal clinical quality and financial outcomes.
Works collaboratively with the Case Management Coordinator/RN and other health care team members.

Ideally, you will have previous hospital experience.


  • This is a PRN position
  • Committment to New Employee Orientation and Orientation to the position is required.
  • Weekend work is required


  • Maryland Social Work licensure required or LSWA
  • LCSW preferred
