The Emergency Department Case Manager is a fast-paced dynamic role combining assessment, planning, facilitation, care coordination, evaluation and advocacy. The Case Manager gathers options and services to meet an individual's and family's comprehensive health needs. The Case Manager communicates with all members of the team, patient, and family members to align resources that promote safety, quality outcomes, and cost efficiency. Successful candidate will use their clinical knowledge and care coordinator experience to prioritze cases and align transition planning to promote efficiency alon the care continuum.

Full-time, 8:00 a.m. - 8:00 p.m.

Schedule includes (3) 12 hour shifts and (1) 8 hour shift - 8:00 a.m. - 4:00 p.m. every two weeks

Weekend Work: Every 4th weekend

Work Location: Johns Hopkins Hospital, East Baltimore Campus, Baltimore, MD 21287

Education:

Level I: Bachelor's Degree from an accredited School of Nursing

Level II: Master's Degree from an accredited School of Nursing

Required Licensure, Certification, Etc.

Both Level I & II require current Registered Nurse licensure in the State of Maryland.

Work Experience:

Level I - Three (3) years of full-time nursing experience.

Level II - Five (5) years of full-time nurisng experience.

Level I & II - Additional case management/disease management or managed care experience preferred. One (1) year of Emergency Room experience preferred.
