The Inpatient Case Manager is a unit-based dynamic role combining assessment, planning, facilitation, care coordination, evaluation and advocacy. The Case Manager gathers options and services to meet an individual's and family's comprehensive health needs. The Case Manager communicates with all members of the team, patient, and family members to align resources that promote safety, quality outcomes, and cost efficiency. The Case Manager facilitates, participates in daily interdisciplinary rounds and uses their clinical knowledge and care coordination background to prioritize cases and align transition planning to promote efficiency along the care continuum.

Full-time, Day Shift

8:00 a.m. - 4:30 p.m.

Weekend Work Not Required

Bachelor Degree from an accredited School of Nursing;

Licensure, Certification, Etc.

Current Registered Nurse licensure in the State of Maryland;

Registered Nurse Case Management Certification preferred (CCM or ACM);

Work Experience:

Three (3 ) years of full-time Nursing experience;

More than one (1) year of Case Management experience preferred;
