Position Summary:

This position will provide Nurse Case Manager coverage for Adult Surgery on 11 West. This position works in collaboration with additional Nurse Case Management, and Social Work partners. Responsibilities include early identification, high-risk screening and readmission screening of all patients. Services include but are not limited to adjustment to illness support, discharge planning, identification of obstacles and barriers to discharge through proactive screening and assessments, and care coordination with the multidisciplinary team. This position also has shared responsibilities for coverage during absences for inpatient units and outpatient clinics within the Department of Care Management which falls under the Department of Care Coordination and Clinical Resource Management, which is approximately 600 beds. Additional areas of focus include length of stay, and throughput initiatives, weekend on-call and emergency preparedness operations.

Education:

Completion of an accredited RN Nursing Program; BSN preferred

Work Experience:

• Minimum three to five years of nursing experience required
• Preferred experience in case management, discharge planning or utilization review

Licensure/Certification:

  • Valid Maryland Nursing License required
  • Accredited Case Manager – Registered Nurse (ACM – RN) / Certified Case Manager (CCM)preferred

Knowledge/Skills:

  • Ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations with ability to form and maintain positive, collaborative relationships with hospital staff, patients, families, post-acute providers and payers.
  • Ability to effectively problem solve in a proactive, creative manner, using judgment based on clinical knowledge.
  • Ability to function effectively in a fluid, dynamic, and rapidly changing environment.
  • Proficiency with an electronic patient record, email, general office and communications equipment.
  • Business acumen so that health benefits and patient/ provider satisfaction are maximized while solidifying the program’s viability through cost effective intervention.
