Johns Hopkins Community Physicians is looking for an experienced Case Manager to work at our East Baltimore Medical Center practice in Baltimore, Maryland. The Case Manager, embedded in the community-based, primary care setting, works as part of the interdisciplinary team to anticipate and coordinate care / services / health education for targeted member populations with chronic disease conditions or high cost specialty care. She/he demonstrates knowledge of complex case management, prognosis and outcomes, and is responsible for ensuring that member goals are achieved within a specified time period based upon individual needs, reasonable expectations and well-documented outcomes. The Case Manager will demonstrate knowledge of the impact of Social Determinants of Health, and have experience addressing psychosocial needs in the medical context. The Case Manager will work with various patient populations across the clinic, including patients from adult internal medicine, pediatrics, and OBGYN practices.
Location: 1000 E. Eager St., Baltimore, Maryland 21202
Shift: Exempt, Full-time - 40 hours per week
Requirements:
- Bachelor’s Degree from an accredited school of nursing/related field; or RN currently enrolled in a bachelors program with a documented plan for completion. Equivalent experience may be substituted for a degree.
- OR Social Work Master's Degree in Social Work required.

Demonstrated clinical expertise. Knowledge of health care and managed care delivery systems. Standards of medical practice. Insurance benefit structures and related legal/ medical issues. Utilization management and quality improvement processes. Community resources. Clinical, satisfaction and financial outcomes.

Current registered nurse licensure in the state of Maryland. Certification in case management (CCM) preferred. For Social Work: Current Maryland licensure with the Board of Social Work Examiners LCSW.

Minimum five (5) years clinical practice. Three (3) years case management. Managed care experience preferred.
