The position provides direct patient care in the Bartlett Specialty Practice. Provides direct psychological, social and practical care to a select caseload of patients and families including social work assessments, problem identification, goal setting and implementation of care plans. Makes referrals to outside agencies and coordinates continuity of care. Provides critical services that enables patients to enter and maintain medical care to treat their HIV/AIDS diagnosis. Works collaboratively with the multidisciplinary team to optimize patient outcomes. Contributes to the attainment of organizational goals. Will be required to provide coverage during absences, vacancies and on call

Education: What is the minimum level of formal education required - MSW degree from an accredited school of social work.

Knowledge: What are the acquired information or concepts needed - Knowledge of biopsychosocial and cultural elements of clinical social work practice. - Knowledge of procedures, systems, and organizational objectives used within social work. - Effective knowledge of community resources. - Knowledge of current health care environment. - Knowledge of and ability to gather relevant data to synthesize and summarize information in making judgments regarding patient care.

Required Licensure, Certification, Etc.: Must be licensed by the Maryland Board of Social Work Examiners at the LGSW level. -

Work Experience: 1 year of experience in providing clinical social work service, preferably, in a mental health/health care setting (internship may apply).

40 hours per week

8:30am - 5:00pm

Work location: Ryan White Services, John Bartlett Speciality Practice

Possible exposure to communicable diseases and hazardous materials with little likelihood of harm if established health precautions are followed Works in area that may require contact with aggressive and/or combative patients Works in normal office environment where there is little discomfort to due to the extremes of heat, dust, noise, temperature and the like
