Position Summary:

The Care Coordination team will pilot 6 SW dedicated to the DOM/SOS . This will reduce discharge delays/decrease length of stay across the weekend and into the beginning of the week. This position provides more staff, which will allow the team to assess new patients, and monitor if these patients are able to discharge on Monday/Tuesday. Provide email handoff reports of all patients; Staff will attend daily huddle to discuss cases in need of attention.

Knowledge:

Knowledge of biopsychosocial and cultural elements of clinical social work practice.

Knowledge of procedures, systems, and organizational objectives used within social work.

Effective knowledge of community resources.

Knowledge of current health care environment.

Knowledge of and ability to gather relevant data to synthesize and summarize information in making judgments regarding patient care.

Knowledge of the medical and psychosocial sequelae of course of illness and impact on social functioning (may be specific to patient population).

Knowledge of relevant community resources commensurate with illness, symptoms, mental status and impairments in functioning.

Education:

MSW degree from an accredited school of social work.

Work Experience:

2 years or more of licensed clinical social work experienced in a mental health/health care setting. Internship may be considered.

Will Consider level I- 1 year experience, LMSW

Shift:

Day Shift, Part time (24 hours)

8:00 am – 8:00pm

Weekend work required

2- PT weekend only positions (24 hours week) Saturday and Sunday only

Exempt, Paygrade: CK

Work Location:

JHH, 600 N. Wolfe Street

Skills:

Excellent oral and written communication skills necessary to communicate/exchange and interact effectively and professionally with hospital staff, agency health care professionals, patients and families.

Proficiency and skill in providing social work services in a mental health/health care setting.

Analytical ability to assess patient needs, develop associated treatment plans and deliver effective interventions.

Required Licensure, Certification:

Must be licensed by the Maryland Board of Social Work Examiners at the LCSW or LCSW-C level.

Exceptions:

Is licensed to practice social work in another state or possesses social work qualifications in another jurisdiction comparable and

Meets requirement established by the Board in regulations and

Has an application for a license pending before the Board or

Has successfully passed an examination or examinations prescribed by the Board pertinent to the license sought.

Working Conditions:

Work frequently produces high level of mental fatigue.

Works in a normal office environment where there are limited physical discomforts.

Must be able to work in a challenging, fast paced environment.

Possible exposure to communicable diseases, hazardous materials, and pharmacological agents.

Possible exposure to violence.
