The Licensed Clinical Social Worker (LCSW-C) is a key team member of the Johns Hopkins Home-based Medicine (JHOME) program. The JHOME program is a primary care program that provides home medical visits for older patients who can no longer reach a medical office. The LCSW-C will be responsible for providing psychosocial support to chronically and acutely ill older patients, their caregivers and families. Performs crisis intervention, end-of life care planning and goal setting, short-term counseling and referral to appropriate rehabilitation, community programs, and agencies. Assesses patients and caregivers for mental health illnesses such as depression, anxiety and stress while utilizing screening tools and coping strategy education. Provide home safety assessments, when needed. Works with the interdisciplinary team in integrating social work services into the plan of care and provides direction to the Community Health Worker. The LCSW-C manages education materials related to community resources and ensures they are consistently updated as new programs/services become available. The LCSW-C is a valued member of the JHOME team and is expected to fully participate in both administrative and patient care team meetings.
