This position will provide PT social work services to the Adult Emergency Department. Responsibilities include crisis intervention, early identification, high risk screening and readmission screening of all patients. Services include but not limited to adjustment to illness, grief and loss counseling, critical care support and intervention during sudden traumatic events, safety assessments, discharge planning, identification of barriers and obstacles to discharge, and coordination of care with multidisciplinary care team. This position is within the Department of Medicine and Surgery Social Work which falls under the Department of Care Coordination and Clinical Resource Management. Additional areas of focus include integration with Adult Psychiatry Consult Service, throughput initiatives, capacity management operations, weekend on call and social work emergency preparedness.
