This position will provide social work coverage for the Adult Medical Intensive Care Unit and the Medical Progressive Care Unit. The MICU is a 24 bed inpatient unit and the MPCU is a 21 bed inpatient unit. Responsibilities include early identification, high risk screening and readmission screening of all patients. Services include but are not limited to adjustment to illness, grief and end of life counseling, safety assessments, discharge planning, identification of obstacles and barriers to discharge, and coordination of care with multidisciplinary team. This position has shared responsibility for social work coverage for all inpatient units within the Department of Medicine and Surgery Social Work which falls under the Department of Care Coordination and Clinical Resource Manangement and encompasses approximately 600 beds. Additional areas of focus include length of stay, throughput initiatives, weekend on call and social work emergency preparedness operations.

Shift: 8:30 - 5pm
Weekend/Holiday On Call Rotation
Weekend Onsite work voluntary for supplemental pay.

Education: Masters degree in Social Work
Level I- LMSW
Level II- LCSW-c
Experience: At least 1 year experience required, internship will be considered.
