Clinical Pharmacy Specialist – Ambulatory Care (Primary Care and Telemedicine):
The Johns Hopkins Hospital is seeking a motivated individual to assume a clinical position in ambulatory care practice. The individual will work with interdisciplinary teams in primary care clinics to provide comprehensive medication management, initiate and optimize pharmacotherapy, support transitions of care services, provide patient education, precept residents and students, and participate in population health and research activities. The individual will work with the Johns Hopkins Maryland Primary Care Plus care model to provide direct patient care via telemedicine video and telephone visits and direct face-to-face patient interactions. The Clinical Pharmacy Specialist will provide chronic disease state management for primary care disease states, such as hypertension and diabetes.
They will serve as a preceptor for pharmacy residents of ASHP accredited PGY-1 and PGY-2 residency programs. Faculty appointments are available through affiliations with the University of Maryland College of Pharmacy, the University of Notre Dame of Maryland College of Pharmacy, and the Johns Hopkins University School of Medicine.
