Johns Hopkins Community Physicians is looking for a Clinical Transition Specialist in the Central Services Department in Baltimore, MD. Clinical Transition Specialist provides for the safe and effective transition of patients through the continuum of care. In this new role the Clinical Transition Specialist facilitates patient safety and quality health outcomes and contributes to designated quality measures. Offers friendly, courteous assistance to every customer to ensure that they have a positive, professional experience with JHCP. You will make appropriate decisions and takes appropriate actions using available resources, to achieve positive outcomes for patients. Communicates decisions to appropriate management.

Location: 3910 Keswick Rd.; Baltimore, MD 21211

Shift: Non-exempt. - Full-time - 40 hours per week - Mon-Fri - 8:30 - 5:00 p.m.


  • High school degree or GED equivalent is required. Graduation from an accredited clinical training program preferred.
  • Five or more years of specific applicable clinical experience in medical care setting may be substituted for formal training program is preferred.
  • Two years’ experience in an ambulatory care setting is preferred.
