CNA , Care Manager -Grand Oaks Assistant Living

Eligible for $1000 Sign on Bonus

The CNA - Care Manager is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living neighborhoods. This Universal Care Manager role includes providing hands on care, physical, and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. The Care Manager is responsible for demonstrating the Mission for Grand Oaks, " to champion quality of life for all seniors" in accordance with federal, state and local standards, and regulations and Grand Oaks policies to promote the highest degree of quality care and services to our residents.

Location: Washington D.C.

Full time nights - 11:00pm - 7:30am

36 hours per week

Minimum Requirements:

1. High school diploma/GED accepted.
2. Current certification as CNA or HHA in the District of Columbia.
3. Must be at least 18 years of age.
4. Previous experience working with seniors preferred and desire to serve and care for seniors.
