Grand Oaks is a beautiful Assisted Living community nestled in the tree-lined streets of Northwest Washington, D.C. on the campus of Sibley Memorial Hospital. The building is a Georgian style residence with spacious apartments and variety of services to help every resident from independent living through memory care for those living with memory disorders and Alzheimer’s disease. The Care Manager is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living and memory neighborhoods. This Care Manager role includes providing hands on care, physical, and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. The Care Manager is responsible for demonstrating the Mission for Grand Oaks in accordance with federal, state and local standards, and regulations and Grand Oaks policies to promote the highest degree of quality care and services to our residents.

Minimum Qualifications: Diploma/Certificate/GED required. Current District of Columbia CNA or HHA license.

8 hours per week

Shift: Rotating
