Grand Oaks is a beautiful Assisted Living community nestled in the tree-lined streets of Northwest Washington, D.C. on the campus of Sibley Memorial Hospital. The building is a Georgian style residence with spacious apartments and variety of services to help every resident from independent living through memory care for those living with memory disorders and Alzheimer’s disease. The Lead Care Manager is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families including leading the assigned tasks of Care Managers. This Lead Care Manager role includes providing hands on care, physical, and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe, comfortable home like environment. The Lead Care Manager is a responsible for championing quality of life for all seniors in accordance with federal, state, and local standards and regulations and Grand Oaks policies to promote the highest degree of quality care and services to our residents.
Minimum Qualifications: Diploma/Certificate/GED required. Current District of Columbia CNA or HHA license.
36 hours per week
