General Summary: The Dementia Support Associate is responsible for supporting digital and telehealth connections with both active and potential Club Memory® members as well as supporting provision of high quality dementia specific assessment and programming. The Dementia Support Associate also supports community education, resource acquisition and communication to qualified members of the community, displaying good skills in assessment, program planning, and individual treatment in the promotion of health in Senior Association and Club Memory members.

Minimum Education and Clinical Experience Required:

  1. Bachelor’s Degree preferred.
  2. Nurse, Social Worker, Counselor, Family Therapy or Recreation Therapy experience preferred.
