Provides leadership for Care Coordination within the Office of Johns Hopkins Physicians (OJHP) by conducting strategic planning, directing, coordinating, and managing the Care Coordination functions to facilitate the provision of seamless high-value care for patients across the continuum, with particular focus on care coordination functions in ambulatory care. Responsibilities include: Contributing to the mission and goals of the Johns Hopkins Medicine by participating in strategic initiatives, assuring compliance with all applicable federal, state, and local regulatory agencies and programmatic requirements, leading and managing the care coordination team, and development of partnerships with other Hopkins affiliates and other external partners.

Work requires completion of a Bachelors of Nursing along with a Master’s degree in Nursing or a closely related field. Minimum ten (10) years clinical practice. Minimum three (3) years in a Director role related to Care Management or Care Coordination. Previous experience in building, leading, growing, and managing care coordination programs required. Prior experience in Strategic Planning and implementation of programs related to care coordination or value-based care. Current registered nurse licensure in the state of Maryland. Certification in case management (CCM) preferred. Supervises all OJHP clinical care managers. Assigns work to support staff.

Day Shift - 40 hours

Shift: 8:30 to 5:00pm

Pay grade: MM

Work is generally sedentary in nature; however, some standing, stooping, and bending is required. Work requires concentration and constant attention to accuracy and detail for extended periods of time. Work requires cross-training and coverage of colleagues. Work requires daily use of a computer. Work requires regularly interacting and meeting with site leaders to ensure timely and consistent access to care coordination and psychosocial services for all patients, including transitional care planning and psychosocial assessment and intervention. Work requires visits to provider offices and inpatient facilities. Work requires attendance at company office and off-site meetings.
