As the Director of Care Coordination, you will work with an amazing team of Social Workers (Discharge Planning) and Registered Nurses (Utilization Review) and direct all care coordination activities at Suburban Hospital.

Our ideal candidate will have the following:

  • Director - level leadership in hospital setting in Care Coordination
    • Looking specifically for someone who enjoys mentoring
  • Thorough understanding of reimbursement (preferably in the state of Maryland) and how it impacts utilization review and coordination of care
  • Ability to analyze and present data (and then establish programs and procedures), to include
    • Lengths-of-stay, readmissions, denials, and clinical outcomes


​​​​​​​The ideal candidate will have the following experience/education/licensure:

  • ​​​​​​​5+ years' experience in Utilization Review, Clinical Pathways, Case Management, and Disease Management
  • Progressive Leadership roles in Care Coordination
  • Bachelor's Degree required.
  • Master's Degree in Nursing or Healthcare Administration, or Business required or commitment to enroll in 3 months
  • Maryland RN licensure (or other Compact state)
  • Certification in Care Coordination is preferred
