JOB SUMMARY:
- Plans, organizes, implements, & directs care coordination activities at Suburban Hospital.
- Provides oversight and broad direction to Care Coordination, including care coordination, utilization review, social services, and discharge planning.
- Works closely with medical staff and Hospital leadership to identify patient care trends & issues.
- Develops care management programs, procedures, and clinical pathways that ensure efficient use of medical resources and provide optimal cost-effective, quality care.
- Responsible for providing leadership, direction and strategic vision for population health strategies across the organization
- Establishes strong partnerships in the community and leverages the care coordination team to support population health goals.
EDUCATIONAL REQUIREMENTS/PREFERENCES:
Bachelor of Science in Nursing
Masters in Nursing or Healthcare Administration preferred
EXPERIENCE/REQUIREMENTS:
- Five to seven years’ experience with utilization review, clinical pathways, case management, and disease management
- Previous supervisory experience
