The Director of Population Health reports to the Vice President of Population Health and Advancement.
Role Accountabilities Include

Direct, plan for and oversee deployment of interventions tied to population health goals and ensure appropriate coordination across care settings;

Provide oversight and direction for interventions funded through HCGH health promotion, behavioral health, advanced care planning and infrastructure investments, including readmissions initiatives;

Develop and maintain strong partnerships across key units across the hospital and in the community; and

Direct day-to-day operations and management for the Howard County Regional Partnership (aka Howard Health Partnership).
The Director is responsible for providing leadership, direction and strategic vision for population health strategies across the organization. The Director will build strong relationships with internal hospital departments such as nursing, social work and case management and help to strengthen community-based partnerships. The individual in this position must be knowledgeable in the areas of health care delivery systems, social determinants of health and health care transformation, and have an understanding of relevant Federal and State regulatory requirements, including the Maryland All Payer Model.
EDUCATION & EXPERIENCE:
- Masters’ degree or higher required in business administration, health economics, health care administration, public health, or closely related field.
- A minimum of 5 years progressively responsible professional level administrative management/leadership and financial experience.
- Ideal candidate has experience working with health systems, hospitals, physician groups, and community-based organizations and experience working with and collaborating successfully with senior hospital and community leaders, and experience in preparing and presenting information to leadership.
