Reporting to the Transition and Patient Experience Sr. Director, Home Care Coordinator/Discharge Planning (HCC/DCP) & Liaison Manager is a registered nurse who has a thorough understanding of available home care services and products, eligibility requirements, and resources available in the community and manages the functions within designated teams. The Manager acts, in a leadership capacity, as a discharge planning resource to Hospital staff involved in home care planning, and facilitates continuity of care for the patient referred to receive home care services. Responsibilities include overseeing day-to-day operations and supervision of staff, while also ensuring adequate staffing at designated sites. This role integrates into the health care system by serving to ensure patients receive the appropriate home-based services to facilitate safe transitions and a positive patient experience. Specifically for Liaison staff, the Manager identifies opportunities to promote services and ensures target are met.
- BSN required. MSN preferred.
- RN in the state of Maryland and CPR
- Two (2) years home care experience required with progressive management.
- Demonstrated excellence in nursing assessment, home care planning and problem-solving skills.
- Demonstrated clinical expertise in many clinical areas, i.e. oncology, infusion therapy, pediatrics.
