The Home Care Coordinator/Hospital Discharge Planner (HCC)/Intake float nurse is a licensed nurse who has a thorough understanding of available home care services and products, eligibility requirements, and resources available through Johns Hopkins Homecare Group and in the community. This position is a float nurse between central intake and JHH , coordinating the discharge of patients needing home care services for all lines of business from JHH and central intake. This nurse also understands the EPIC intake work flow and assists in both areas as back fill as the needs in staffing dictate.
