JOB SUMMARY

Employs the nursing process to develop and implement plans of care for patients requiring in-home health and support services. In addition to specific and skilled treatments and procedures, the nurse provides health teaching to patients and care givers, preventive health screening, referral and coordination of services in the agency and the community, and advocacy for the patient in obtaining health-related goals. The Case Manager ensures adequate service provision, including scheduling of visits, based on patients' identified needs and priorities for care. Complies with all Home Care policies and procedures.

JOB REQUIREMENTS

  • Associate in Nursing required; BSN preferred
  • RN in the state of Maryland
  • Active CPR
  • Critical care experience required; home care experience preferred
