Summary:

This HYBRID position offers great work flexibility. The Intake Nurse (LPN) will be responsible for initiating and processing patient referrals to assure that patient needs are met and agency policies are followed. Establishes and maintains ongoing relationships with referral sources, payors and case managers.

Education:

Completion of an accredited LPN program.

Required Licensure, Certification:

Maryland Licensure as a LPN

Work Experience

3 years of clinical nursing experience, 2 years home care experience preferred.

1-year work experience in a position of responsibility (administration/ supervision/ preceptor) preferable in a case management, community health or home health setting

Home medical equipment and infusion experience preferred.

Knowledge of Medicare and Medicaid, and other 3rd party payer requirements.
