The Lead Case Manager is responsible for supervision and oversight of the case management department and serves as an expert clinical practitioner. As a team leader, the Lead Case Manager provides daily management of personnel issues and staffing for at least 30% of their time. Under the direction of the Manager, Case Management serves as a coach and advisor to staff, a resource, a preceptor to new employees, a reviewer of clinical quality through audits, and follows through on quality improvement initiatives utilizing lean methodology. The Lead Case Manager is also responsible for implementation of the plan of care for specific patients, beginning on a pre-admission basis and following through placement and into follow-up/after care and is empowered to facilitate mid-course care changes, in conjunction with the patient care team. The Lead Case Manager collaborates with the patient, family, Interdisciplinary Care Team members and external healthcare providers to assure safe, timely and efficient care. Position requires innovative skills for continuous improvement in patient outcomes and cost effectiveness. The Lead Case Manager has responsibility for initiating the plan of care and monitoring its implementation. Internal communication is required outside of the work group to persuade or gain cooperation and acceptance of ideas. Job has significant accountability for ensuring internal contact satisfaction. Communication with external contacts may include resolving complaints, making adjustments or correcting mistakes about a selected service or capability. The Lead Case Manager functions as a team leader in the coordination of care to patients and serves as a problem solving resource either on a unit based or on physician based basis.
