Promotes and facilitates community-based screenings and classes for chronic disease prevention and management. Under the direction of the Health Promotion Program Manager, this role conducts chronic disease prevention programming within the community, including faith-based organizations Coordinator will work closely with the Director of Spiritual Care to identify opportunities to expand screenings and classes within faith organizations. The Population Health Program Coordinator is responsible for organizing screenings for hypertension, pre-diabetes and obesity and referring eligible participants to evidence-based community programs. This role will also be responsible for organizing and delivering chronic disease self-management programs across Howard County.
40 hours per week
8:00 AM - 4:30 PM
Some weekend work required based on screening and class schedules.
Work Location: Howard County General Hospital
Bachelor's degree in Public Health, Community Health or other health science program required; Master's degree preferred.
