Johns Hopkins Health Systems is seeking a RN Care Manager in Hagerstown, MD, Westminster, Charles County, Montgomery County, and Annapolis/Bowie areas. The RN Care Manager is an integral member of the primary care team, tasked with leading efforts to engage patients and coordinating care for high risk and high need patients with complex health care and social needs. The Care Manager, working alongside the primary care provider, proactively anticipates and coordinates care services across multiple providers and care settings and provides comprehensive self-management and health education for targeted member populations with chronic disease conditions or high cost specialty care. She/he demonstrates knowledge of complex care management core competencies, disease prognosis and outcomes, and is responsible for ensuring member goals are achieved within a specified time period based upon individual needs, reasonable expectations and well-documented outcomes.

Bachelor’s Degree from an accredited school of nursing/related field; or RN currently enrolled in a bachelors program with a documented plan for completion. Equivalent experience may be substituted for a degree. Advanced - Master’s Degree from an accredited school of nursing / related field required or Bachelor’s degree and currently enrolled in a graduate program with documented plan for program completion.

Associate - Minimum two (2) years clinical practice. One (1) year care management. Managed care experience preferred.

Care Manager - Minimum five (5) years clinical practice. Three (3) years care management. Managed care experience preferred.

Advanced - Minimum eight (8) years clinical practice. Three (3) years care management/ disease management experience in a managed care organization.

Current registered nurse licensure in the state of Maryland. Certification in case management (CCM) preferred.

Day Shift - 40 hours

Shift: 8:00 to 5:00pm

  • Associate Care Manager: CK
  • Care Manager: CL
  • Advanced Practice Care Management: CM

Work is generally sedentary in nature; however, some standing, stooping, and bending is required. Work requires concentration and constant attention to accuracy and detail for extended periods of time. Work requires cross-training and coverage of colleagues. Work requires daily use of a computer. Work requires regularly interacting and meeting with site leaders to ensure timely and consistent access to care coordination and psychosocial services for all patients, including transitional care planning and psychosocial assessment and intervention. Work requires visits to provider offices and inpatient facilities. Work requires attendance at company office and off-site meetings.
