POSITION SUMMARY:
The professional RN care Coordinator provides patient centered, specialized, evidence based nursing care across the continuum through an inter-professional approach to treatment, research, education and advocacy. Responsible for ensuring the Care Management requirements for PCMH are met. Duties include bridging the gap between inpatient and outpatient care for the medically complex patient. Contributes to the goals of the department by being accountable for the delivery of compassionate and safe care within the scope of practice as defined by the Florida Board of Nursing and JHACH policy.
Work Location: St. Petersburg, FL
Work Hours: M-F (8a-4:30p)
QUALIFICATIONS:
- Current and active Florida RN license
- BSN
- 5 years’ experience with Care Coordination
- BLS certification, AHA
- Knowledge of computer systems
- Ability to read, speak and understand English
