Position Summary

The Senior Director for Care Management is responsible for the development, direction and administration of the care management model implemented to ensure the provision of high-quality care and facilitation of transitions that are coordinated, timely and patient- and family-centered. The Senior Director oversees all JHH Social Work and Case Management services, providing the strategic direction and guidance required to deliver exemplary service while anticipating and adapting to changes in the healthcare delivery and regulatory environment. The Senior Director ensures that state and federal regulatory, accreditation and compliance standards related to care coordination and transitions planning are met and that policies and processes are created/amended as needed to fulfill requirements. The Senior Director develops and coordinates programs with community service organizations and works in partnership with departmental leaders to implement initiatives designed to address JHM strategic priorities related to patient and family centered care.

Competencies:

Education:

Master’s degree in Social Work or Nursing

Knowledge:

In-depth knowledge of healthcare environment, regulatory and compliance standards, care management systems, and social work and case management standards

Exceptional understanding of the structures, procedures, and organizational priorities used to facilitate social work and care management processes at all levels of care

Knowledge of integrated health care delivery systems, and experience in community planning and interagency collaboration

Extensive knowledge of leadership and management principles, including budgeting, human resource planning, and other operational techniques required to run a large clinical department

Skills:

Excellent communication and interpersonal skills – including ability to serve as consultant to leadership/healthcare teams regarding CM/SW and the critical services provided by these disciplines

Effective leadership and management skills, including coaching and mentoring

Excellent critical thinking and demonstrated ability to analyze, synthesize and summarize information for decision making

Expertise in data analysis, outcomes management and effective performance improvement strategies

Ability to influence and negotiate individual and group decision making

Required Licensure Certification:

Active licensure in field:

Social Work – Current licensure as Certified Social Worker Clinical (LCSW-C) by the State of Maryland

Nursing – Current licensure as Registered Nurse (RN) by the State of Maryland and certification in Case Management

Work Experience:

10 years of progressively responsible social work or nursing administrative experience, preferably within an academic medical center

Must have a demonstrated record of accomplishment in social work administration, clinical practice, and innovative practice delivery

Day Shift, Full Time 8:30 am – 5:00 pm

Exempt, Pay Grade: AD

Work Location, JHH 600 North Wolfe Street

Working Conditions:

Works in normal office environment where there are no physical discomforts due to temperature, noise, dust and the like.

Work frequently produces a high level of mental/visual fatigue due to attention to details and adherence to deadlines
