Summary
Reports to an assigned Licensed Certified Social Worker. Responsible for assessing the needs of clients and providing counseling or referral to appropriate community service agencies. May provide clinical supervision to social work students. Incumbents may be assigned to specific clinical specialty. Provides relevant input in the development of new Social Work services, and/or improvements to health care delivery systems.
Principal Duties and Responsibilities
1. Evaluates the problems of clients, hospital or clinic patients.
2. Interviews clients, hospital or clinic patients to obtain information.
3. Provides counseling to patient and family.
4. Consults with other professionals in following up recovery, welfare or rehabilitation of clients and patients.
5. Initiates and completes appropriate and timely discharge planning.
6. Provides information and referral services.
7. Participates in a variety of conferences, meetings and staff development sessions.
8. Maintains necessary records and statistics.
Required Knowledge, Skills, and Abilities
1. Work requires the knowledge of theories, principles, and concepts typically acquired through completion of a Master’s degree in social work and less than six months of previous experience.
2. Current state licensure as LGSW required.
3. Work requires the analytical skills to resolve problems that require the use of basic scientific, mathematical, or technical principles.
4. Work requires the ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations.
