Day Shift; 10:30am - 7:00pm


Position Summary:

The primary focus of the social worker is to provide abroad range of discharge planning services to patient and their families. This enhances inpatient care and lays foundation for continuing outpatient care. Works mostly with patients 18 years and older. Must be knowledgeable however about adolescence as a developmental stage since patients under 18 may be admitted. Because of the increasing admissions of elderly chronically mentally ill must be aware of aging as a developmental process. Provides direct care to a select caseload (i.e. clinically complex) of patients and families including assessments, problem identification, goal setting and intervention plans and implementation of plans. Makes referrals to outside agencies and discharge planning as needed. Serves as an expert in a specialty area. May provide field instruction to a graduate student or peer supervision to BSW or LGSW.

Education/Work Experience:

Work requires the knowledge of theories, principles, and concepts typically acquired through completion of a Master’s degree in social work and two years of post-masters experience providing clinical social work, preferably in a health care setting.


Current state licensure as LCSW required.

Knowledge, Skills and Abilities:

  • Work requires the analytical ability to assess patient needs at a micro and macro level, develop associated treatment modalities at a more complex level and provide affective counseling.

  • Work requires the ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations.
