The weekend ED Social Worker is responsible for assessment, care coordination and resource referral for patients experiencing trauma, intimate partner violence, homelessness, substance use d/o, abuse and neglect, as well as post acute and group home placements. The successful candidate will perform psychosocial assessments and will develop mutually agreed upon intervention plans with patients and families. This will include assessment for substance use, anxiety, depression, and/or adjustment to illness. Will provide referrals to community agencies that support efforts to guide health maintenance and optimal coping. Must be able to work collaboratively on a multidisciplinary team in a fast paced environment. Experience should include direct practice in hospital med/surg setting.
Every other weekend, Saturday and Sunday from 11am to 7pm
Education: Master's degree in Social Worker
Experience: 2 years related experience
