Johns Hopkins Community Physicians is currently seeking a full-time Social Worker MSW at East Baltimore Medical Center Practices. Social Worker provides clinical Social Work services throughout the JHCP clinical sites. Provides education, crisis intervention, short term counseling, planning for long term care placement, and information and referral services to community and government placement agencies. Documents all social work interventions, assessments, plans and follow-ups in the appropriate format for inclusion in the medical record chart.

  • JHCP Community Physicians - EBMC
  • Full-time - 40 hours per week
  • Masters Degree required
  • Current Maryland licensure with the Board of Social Work Examiners or license eligibility at LMSW, LCSW or LCSW-C level.
  • One year of post-graduate Social Work experience in a health care setting, preferably ambulatory care.
