Johns Hopkins Community Physicians is currently seeking a full-time Social Worker MSW at East Baltimore Medical Center. The Social Worker will provide clinical Social Work and medical case management services in a grant-funded position to support the Med-Peds practice. Provides education, coordination of care, resource and referral to community agencies. Documents all social work interventions, assessments, plans and follow-ups in the appropriate format for inclusion in the medical record chart.
Location: 1000 E. Eager St., Baltimore, Maryland 21202
Status: Exempt, Dayshift - Part-time 30 hours per week
Requirements:
- Master's degree in Social Work is required.
- Current Maryland licensure with the Board of Social Work Examiners or license eligibility at LMSW, LCSW or LCSW-C level.
- One year of post-graduate Social Work experience in a health care setting, preferably ambulatory care.
