The Social Worker PRN provides social work intervention to patients on the Inpatient Med Surg, Emergency Department and Maternal Child Health units. This will include psychosocial assessments, transition planning to home, inpatient hospice, post-acute rehab and assisted living. The social worker will follow up with patients from a sign out list that is generated on Friday and will see any new referrals over the weekend for discharge planning. Strong written and oral communication skills are necessary for timely referrals and documentation in the medical record. Must have strong skills in crisis intervention, family systems work, conflict resolution and collaborative teamwork.


Required Knowledge, Skills, and Abilities

Work requires the knowledge of theories, principles, and concepts typically acquired through completion of a Master’s degree in social work and two years of post-masters experience providing clinical social work, preferably in a health care setting.

Current state licensure as LCSW or LMSW required.

Work requires the analytical ability to assess patient needs at a micro and macro level, develop associated treatment modalities at a more complex level and provide affective counseling.

Work requires the ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations.

Required Physical, Mental, and Visual Abilities

Work requires occasionally reaching and grasping objects.

Work requires occasionally using a keyboard to interact with computer system.

Work requires proofreading and/or reviewing documents.

Day Shift,

This position is an 8 hour shift - may include Saturday and/or Sunday

Working Conditions

Work is generally performed in an office or clinical environment in which there is only minimal exposure to communicable disease, hazardous materials and pharmacological agents with little likelihood of harm if established health and standard precautions are followed. Possible exposure to violent behavior.
