Potomac Home Health Care (PHHC) and Potomac Home Support Inc. (PHS) are now proud to be part of Johns Hopkins Home and Community Based Services. As a non-profit joint venture of Sibley Memorial Hospital and Suburban Hospital, our programs have the unique advantage of offering the resources of two full-service hospitals as well as the nationally acclaimed Johns Hopkins Health System.
We are a full service, Medicare certified home health agency and private duty agency providing a comprehensive range of home care services in Maryland and Washington, DC, including mental health, infusion therapy, nursing, physical, occupational, and speech language pathology. Working within a team approach, we make every attempt to recognize and meet the needs of the homebound patient. Our programs stress personalized care, tailored to each patient's individual needs, as we work closely with the physician, the patient and family members in planning, reviewing progress and making adjustments to the plan of care. This approach results in a better treatment plan and greater understanding and support for each patient and their family.

PHS provides a diverse range of private pay services including:
- Certified Nursing Assistants
- Certified Home Care Aides
- Geriatric Care Management Assistants (GCMAs) for transportation assistance
Our PHS team members take pride in providing the personal attention and care patients need in the home, hospital, assisted living or nursing center settings.
