What you will do:
The Assistant Director of Transitions provides operational and programmatic oversight for the Care Management Resource Centers.
Who should apply:
- Bachelor’s degree in a health-related field required; Master’s degree preferred.
- Minimum of five years’ progressive leadership experience in home care with experience in care coordination within a health system.
- Care coordination, discharge planning, or support services in an inpatient hospital setting is required.
- Healthcare professional license preferred.

Salary Range: Minimum 63.98/hour - Maximum 99.17/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
