Responsible for responding to prospective member and provider inquiries generated through advertising and marketing initiatives in accordance with departmental policies and procedures. Assesses members' health and social risk factors to identify care needs and coordinates appropriate care plans and interventions. Assists members in overcoming barriers to accessing healthcare services by arranging transportation, language interpretation, and other support resources while engaging non-adherent members in care. Facilitates member enrollment and ensures enrollees are connected with appropriate healthcare services. Manages an assigned member caseload in compliance with COMAR regulations and Johns Hopkins HealthCare (JHHC) policies and procedures. Coordinates referrals to case management, social work, behavioral health, health education, and other support services based on identified member needs. Supports members through the redetermination process to help maintain eligibility and continuity of coverage. Consistently meets established standards for productivity, quality, reporting, and administrative responsibilities.
Requirements:
Eucation:
Requires High School diploma or equivalent. Associates degree related to advocacy, health care, or community relations preferred.
Knowledge:
• Requires basic knowledge of social and health issues to effectively determine appropriate educational and service support.
• Requires familiarity with the personal experiences of families with low/moderate income.
• Requires a working knowledge of local, state, and federal family support resources.
• Familiarity with medical terminology, ICD-9/10 and CPT codes beneficial.
• Knowledge of DHMH eligibility and redetermination requirements.
• Requires experience engaging and working with community stakeholders.
• Requires experience as an advocate for groups or persons that are underserved.
Skills:
Salary Range: Minimum 20.52/hour - Maximum 33.88/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
