What You Will Do:

This position will focus on transitions of care from hospital to home. The ideal candidate will have experience in case management, population health, and readmission prevention, along with a strong understanding of the transitions of care space. Knowledge of insurance payer processes is helpful, as is familiarity with Medicare and Medicaid regulatory requirements. Experience with industry best practices in transitions of care is highly valued. Strong interdisciplinary communication skills are essential, as this role partners with multiple stakeholder teams across the organization.

Qualifications:

• Bachelor of Science in Nursing (BSN) (Required) and Master of Nursing (MSN) (Preferred)
• RN - Registered Nurse License (Required)
• Minimum 5 Years of Experience in clinical nursing (Required)

Salary Range: Minimum 52.76/hour - Maximum 81.78/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
