Position Summary
The Home Care Coordinator Assistant supports all functions related to home care coordination. This role requires knowledge of home care products, services, and community resources.
Responsibilities include:
- Explaining service options and helping patients make informed choices
- Coordinating referrals, home health services, and medical equipment needs
- Communicating with agencies and delivering equipment when needed
- Entering data into electronic medical records
- Participating in rounding
- Handling office support tasks such as:
- Answering phones
- Providing customer service
- Ordering supplies
- Troubleshooting office equipment and submitting service calls
Qualifications
- High school diploma or equivalent required
- Post-high school education or training preferred
- 1 year of data entry experience
- 1 year of customer service experience in a health care setting
- Previous scheduling experience preferred
Physical Requirements
- Ability to lift and carry medical equipment such as infusion pumps, portable oxygen tanks, or walkers
- Ability to stand, walk, or sit for extended periods
Salary Range: Minimum 16.34/hour - Maximum 26.94/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
