What You Will Do:

The Assistant Director of Transitions oversees the day-to-day operations of the Readmissions Department, including staff management, program development, and strategic planning. This role leads efforts to improve care transitions for patients moving from inpatient care back to the community. This role supervises home care coordinators, discharge planners, liaisons, and transition guides, while partnering with affiliates to improve access, patient acceptance, safe care, and efficiency. This position also establishes collaborative relationships with nursing facilities and assisted living facilities and develops strategies to build community capacity to support patient discharges. This role plays a key part in reducing length of stay, preventing readmissions, and enhancing the patient experience.

Who Should Apply:

  • Bachelor’s degree in a health-related field required; Master’s degree preferred.
  • Minimum of 5 years’ progressive leadership experience within a health system is required. Two years’ nurse management experience on an acute care inpatient unit is highly preferred.
  • Healthcare professional licensure is preferred.

Salary Range: Minimum $59.35/hour - Maximum $103.88/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
