Care Management Assistant

Under the direction of a licensed social worker, provides support to social workers by handling routine responsibilities that help increase social work time with families and improve workflow. Specifically, the Care Management Assistant II handles nursing home returns for the medicine service, coordinates contact with Delmarva, and assists with other duties as needed. Requires understanding of the needs of the elderly.

JOB REQUIREMENTS

 Required Knowledge, Skills, and Abilities

• No certification, registration, or licensure required.
• Requires analytical skills to resolve problems using basic scientific, mathematical, or technical principles.
• Requires the ability to exchange factual information, schedule appointments, greet visitors, explain medical center policies, and/or relay messages to appropriate personnel.
• Requires courtesy and tact when dealing with patients, visitors, and/or medical center employees.
• More sensitive situations may be referred to others as appropriate.

Required Physical, Mental, and Visual Abilities

• Requires occasional reaching and grasping of objects.
• Requires regular use of a keyboard to interact with a computer system.
• Requires proofreading and/or reviewing documents.

Salary Range: Minimum $20.52/hour - Maximum $33.88/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
