Social Worker

In collaboration with patients, families, physicians, social workers, and the interdisciplinary team, the Social Worker II provides direct psychosocial care, including assessment, care planning, counseling, and discharge planning. This role supports patients and families by identifying barriers to care, coordinating resources, facilitating communication across disciplines, and providing interventions such as crisis support, grief counseling, and abuse/neglect reporting as needed.

Essential Functions

• Conducts screening and full psychosocial assessments for patients with identified social work needs.
• Develops and implements individualized plans of care and transition plans.
• Provides direct psychosocial support and coordinates care with the multidisciplinary team.
• Identifies and addresses barriers to discharge and access to services.
• Facilitates family meetings and participates in interdisciplinary rounds.
• Maintains timely, accurate documentation in accordance with departmental and regulatory standards.
• Connects patients and families with community and referral resources.
• Assists with case-specific needs such as guardianship, adoption, psychiatric referral, abuse/neglect/IPV assessment, end-of-life planning, and charity care.
• Serves as a clinical resource, mentor, and consultant to staff, students, and other social workers.
• Participates in quality improvement activities and committees as assigned.
• Provides coverage, on-call support, and home visits as needed.
• May provide clinical supervision if approved by the Maryland Board of Social Work Examiners.
• Performs other duties as assigned.

Qualifications

Education:

• Master of Social Work (MSW) required

Licensure:

• Valid LCSW-C required

Knowledge, Skills, and Abilities:

• Strong understanding of community resources and psychosocial impacts of illness
• Ability to work independently in a healthcare or mental health setting
• Strong assessment, counseling, and problem-solving skills
• Ability to develop comprehensive biopsychosocial assessments and treatment plans
• Excellent communication, documentation, and organizational skills
• Proficiency with electronic medical records and standard office systems

Salary Range: Minimum 34.62/hour - Maximum 57.19/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
