Outpatient Pediatric Social Worker
Join our pediatric ambulatory care team as a Social Work II. This role provides clinical, family-centered psychosocial care to children, adolescents, and their caregivers managing acute and chronic health conditions. Responsibilities include comprehensive biopsychosocial assessments; brief therapeutic interventions; crisis assessment and safety planning; support with adjustment to illness, diagnosis, and treatment; caregiver support; advocacy; and collaboration with the interdisciplinary team to address psychosocial factors affecting health outcomes and treatment adherence. The Social Worker applies strong knowledge of healthcare, behavioral health, educational, child welfare, and other public-service systems to help patients, families, and care teams navigate complex needs, identify appropriate next-step supports, and promote continuity of care. The Social Worker will engage and sustain collaborative relationships with community partners to support coordinated care planning, strengthen access to culturally responsive supports, and advance the well-being of pediatric patients and families. Spanish language proficiency is preferred.
Qualifications
Education:
- Master of Social Work (MSW) required
Licensure:
- Valid LCSW-C required
Knowledge, Skills, and Abilities:
- Strong understanding of community resources and psychosocial impacts of illness
- Ability to work independently in a healthcare or mental health setting
- Strong assessment, counseling, and problem-solving skills
- Ability to develop comprehensive biopsychosocial assessments and treatment plans
- Excellent communication, documentation, and organizational skills
- Proficiency with electronic medical records and standard office systems
Salary Range: Minimum 34.62/hour - Maximum 57.19/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
