Community Health Worker
EXPERIENCE / REQUIREMENTS
- High school diploma or equivalent required.
- Must be able to speak, read, and write in English.
- Prior experience in home health, community case management, public health, or a related setting preferred.
- Experience working with community resources, entitlement programs, and third-party reimbursement preferred.
- Bilingual skills highly desirable.
KNOWLEDGE, SKILLS, AND ABILITIES
- Strong understanding of chronic health conditions, behavioral health, and strategies that support self-management.
- Excellent customer service, organizational, and follow-up skills.
- Skilled in motivational interviewing and patient engagement techniques.
- Ability to communicate confidently and effectively with patients, families, physicians, hospital staff, and other members of the health care team.
- Demonstrates professionalism, creativity, discretion, diplomacy, and persistence when working with diverse populations and stakeholders.
- Strong verbal and written communication skills, with the ability to document clearly and communicate effectively by phone, in person, and through electronic systems.
- Proven ability to work collaboratively in a team-based, interdisciplinary environment.
- Able to present information clearly to patients, families, hospital staff, health care providers, and community partners.
- Strong interpersonal skills with the ability to build relationships, support collaboration, and contribute to problem-solving across teams.
- Flexibility to work varying hours as needed to meet department requirements.
- Comfortable navigating information systems and moving throughout the hospital environment as needed.
- Proficient in Microsoft Office applications, including Word, Excel, and Outlook.
Salary Range: Minimum 20.52/hour - Maximum 33.88/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
