Social Worker

Shift: 20-hour, Evening Shift

Make a meaningful difference in patient care

Provide compassionate, patient-centered support to individuals and families as they navigate health and social challenges. You’ll assess needs, develop care plans, connect patients with resources, and partner with clinical teams to support continuity of care.

What you’ll do

• Assess patients’ social, emotional, and environmental needs.
• Develop, implement, and adjust individualized care plans.
• Provide individual, family, and group counseling, as well as crisis support.
• Coordinate care with medical teams, community organizations, and other partners.
• Maintain accurate, confidential case records and documentation.
• Advocate for patients in medical, legal, and other settings.
• Offer guidance to junior social workers and interns.
• Follow organizational policies and perform other duties as assigned.

What you’ll bring

• Master’s degree in Social Work.
• At least two years of experience in social work or a related field.
• Current LCSW or LCSW-C license required upon hire.
• Strong communication, organization, and computer skills; experience with electronic medical records may be required.
• Sound clinical judgment, attention to detail, and a commitment to confidentiality and ethical, patient-centered care.
• Flexibility, adaptability, and the ability to collaborate across disciplines and manage routine to moderately complex cases.

Salary Range: Minimum 34.62/hour - Maximum 57.19/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
