SUMMARY: Responsible for providing Clinical Social Work services to patients on an inpatient Medicine Service
Provides clinical social work services to patients in an inpatient medicine service. Work includes collaborative practice with case management and treatment teams. Responsible for psychosocial assessment including behavioral health, substance use, adjustment to illness and hospitalization, crisis intervention, family meetings, post-hospital planning, and community resource referrals.
Experience working in medical setting preferred
