Shift:

Weekends (Saturday and Sunday)

Day shift; 8-hour shifts

Position Summary:

In collaboration with patients/families, social workers, physicians and the interdisciplinary team, the Case Manager provides leadership and advocacy in the coordination of patient-centered care across the continuum to facilitate optimal transitions and progression in care. The Case Manager manages clinical resources and transition planning for patients within an assigned caseload from pre-admission through post-discharge, actively working to identify/ eliminate barriers to the delivery of clinical services with the patient, family and care team.Coordinates, negotiates, procures, and manages the care of patients to facilitate achievement of quality and cost outcomes. Works collaboratively with interdisciplinary staff internal and external to organization. Participates in performance improvement and evaluation processes related to the management of patient care.

Education/Experience:

Work requires the level of knowledge typically acquired through completion of two years of occupationally specific education or a Bachelor's degree in nursing or a closely related field and two years of previous experience.

Minimum three to five years of nursing experience required.

Licensure/Certification:

Current State licensure as Registered Nurse required.

Valid CPR certification (Healthcare Provider) required.

Certification in Case Management preferred.

Knowledge/Skills:
Work requires the analytical skills to resolve problems that require the use of basic scientific, mathematical or technical principles.
Work requires the ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations.
