Post Acute Care Program Director

Job Summary

The Program Director, Post-Acute Care is responsible for developing and implementing strategies designed to enable the smooth and timely transition of patients from the hospital to lower levels of care. This includes development/oversight of a skilled nursing facility (SNF) preferred partnership, management of Assisted Living Facility (ALF) relationships and assisting with the evaluation, planning, and development of other post-acute programs (e.g. chronic beds, respite services, etc.) that may be required to support the needs of patients discharged from the East Baltimore campuses. This role serves as the hospital’s primary contact for post-acute partners and is responsible for monitoring their performance and ensuring that desired outcomes are achieved. The Program Director closely collaborates with members of the Care Coordination leadership team and works with a broad range of administrative and clinical stakeholders across JHH, JHHS and JHM.

Qualifications

• A Master’s degree in Healthcare Administration or other relevant field is required.
• Minimum 7+ years of progressively responsible related experience working in an operational environment in healthcare. Experience in care management and knowledge of the post-acute care continuum is preferred.
