Johns Hopkins Hospital in Baltimore, MD is hiring for a Social Worker in the Adult Emergency Department (ED).

Schedule:

  • 20 hours per week
  • Shifts are: weekdays 5-11:30pm and weekend 3-11:30pm
  • Required to work every other weekend

Requirements

  • Provide social work support to Adult Inpatient/ Outpatient Rehab Medicine.  

  • Works in collaboration with additional Social Work and Nurse Case Management partners. 

  • Responsibilities include: early identification, high risk screening and readmission screening of all patients.

  • Services include, but are not limited to adjustment to: illness counseling, safety assessments, discharge planning, identification of obstacles and barriers to discharge through proactive screening and assessments, and coordination of care with multidisciplinary team.

  • Shared responsibilities for Care Management coverage during absences for all inpatient units within the Department of Care Management which falls under the Dept of Care Coordination and Clinical Resource Management and is approximately 700 beds.

Qualifications

  • MS Social Work from an accredited school of social work.
  • Must be licensed by the Maryland Board of Social Work Examiners at the LGSW level.
  • Minimum of 1 year experience in providing clinical social work service in a hospital setting is required.
  • Experience with hospital discharge planning is required.
  • Experience working in an Emergency Department is highly preferred.
